Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

· Demonstrates knowledge and appropriate use of the Company Infection Control Manual.

· Understands, participates and assists in implementing the Quality Improvement Program.

· Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.

· Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.

· Conducts family and patient education.

Qualifications

· Currently licensed as RN in state

· One-year prior nursing management experience preferred.

This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
