Position Summary: We have an exciting opportunity to join our team as a APP Manager Park Slope Family Health Centers at NYU Langone. In this role, the successful candidate Responsible and accountable for the management and quality of nursing care delivered at the site as part of the site leadership team. Fosters an effective work environment for interdisciplinary teamwork, patient satisfaction and staff satisfaction. Makes management and budgetary decisions, sets work schedules, recruits and trains nurses and other health care professionals on the care team. Serves as a role model and resource to all staff. Function also includes providing primary care for patients at least one session a week.

Job Responsibilities:

  1. Demonstrates clinical knowledge and skills in area of supervision and directs staff in its application.
  2. Provides direct patient care as needed onsite clinical support/triage/care coordination to meet patient care needs or to model behavior for staff development/training
  3. Conducts daily huddles.  Establishes maintains processes to promote a multidisciplinary team approach in developing, implementing and evaluating patient care and/or family education
  4. Conducts quality improvement monitoring and participates in problem identification and solution to improve key processes/systems/patient care; shares results in staff meetings. Encourages staff participation in quality improvement activities.
  5. Attends and participates in monthly Network meetings
  6. Investigates incidents and occurrences and institutes corrective action as appropriate
  7. Promotes safety and risk free environment for patients and staff
  8.  Ensures staff compliance annual and interval updates, i.e. environments of care, infection control, emerging pathogens,hazard and safety surveillance.
  9. Responds to patient/family needs in timely, positive manner and ensures service excellence among all staff
  10.  Responds to staff, provider and patient satisfaction needs and identifies and implements solutions in collaboration with related departments as needed
  11.   Plans for staffing needs of the site(s)/modules(s) on a long and short term basis, adjusting based on workload and other staffing variables to maintain safe, efficient care in an effective manner
  12.  Prepares staff schedules in a timely manner. Monitors appropriate use of benefit time. Completes time cards for timely and correct payroll. Approves overtime
  13.  Monitors site/module staffing to budget, overtime/agency use and provides justifications as needed
  14.  Interviews potential staff members and recommends hiring of applicants with appropriate skills and behavior in collaboration with recruitment.
  15.  Establishes work performance and clinical competency of personnel on a timely basis and makes recommendations for promotions, transfers and discharges. Takes corrective action and/or disciplinary action with staff in collaboration with Human Resources.
  16.   Facilitates and ensures orientation of personnel to the site/module. Plans for experience needed to perform competently. 
  1.   Participates in the clinical and leadership development of employees in his/her area.
  2.   Establishes work performance and clinical competency of personnel on a timely basis and makes recommendations for promotions, transfers and discharges. Takes corrective action and/or disciplinary action with staff in collaboration with Human Resources.  
  3.   Participates in the clinical and leadership development of employees in his/her area. Maintains accurate records and evaluates effectiveness of activities. 
  4.   Identifies and meets learning needs in order to maintain competency and respond to new clinical and management expectations
  5.   Counsels and coaches staff concerning their work performance and development; holds staff accountable for performance. Makes recommendations for staff educational needs and plans educational activities independently and in conjunction with others.
  6.   Establish pleasant working environment conducive to teamwork, growth and recognition.
  7. Utilizes effective conflict resolution strategies when dealing with staff, providers, patients and family members
  8.   Maintains effective ongoing and timely communication with staff and maintains records of meetings. Makes recommendations for staffing supply and capital budget needs of the module/site. Monitors budget in collaboration with Site Director / Medical Director. Participates in annual staffing/plan budget review process.
  9.  Collaborates with Materials Management Department and Purchasing for appropriate supplies and equipment and ensures proper utilization and maintenance of supplies and equipment on unit
  10.  Assimilates NYU Langone Medical Center/Family Health Centers core values compassion, dignity, integrity, service throughout all processes and interactions
  11.  Participates in module/site/service, departmental and hospital-based councils/activities/special projects
  12.  Serves as a liaison to all other departments in the system and demonstrates effective team building and problem solving strategies in interactions.
  13.  Maintains records on licensure and certifications of the staff

    4.  Works as an independent primary care provider four sessions a week unless otherwise determined by site need.  
    5. Remain current with new developments in your respective specialty.

Minimum Qualifications:
To qualify you must have a Master Degree required. Three years of ambulatory care experience required. Previous leadership experience preferred. Willingness to devote the time required completing assigned tasks on schedule. Valid New York State Nurse Practitioner license, NYS Infection Control Certificate. Basic Life Support (BLS) Certification through the American Heart Association is required at the time of hire and is required to be maintained..

About the Role We are seeking a Nurse Practitioner (FNP) or Physician Assistant (PA‑C) with acute care experience in Emergency Medicine, Urgent Care, or Internal Medicine to join our growing team. As a field‑based Advanced Practice Provider, you will independently deliver comprehensive care in patients’ homes—conducting assessments, diagnosing conditions, managing treatment plans, and coordinating follow‑up care. You will be fully equipped to treat everything from common illnesses to complex injuries, while providing compassionate, whole‑person care across all age groups. The ideal candidate uses strong critical‑thinking skills, follows evidence‑based standards of practice, proactively identifies and solves problems, adheres to all clinical and safety standards, protocols, and performance metrics, and consistently provides care with compassion, empathy, and cultural competency. What We Offer Flexible 10‑hour shifts 4 shifts per week- Full-Time APPs are not required to work weekend shifts. However, weekend shifts are available for those who would like the opportunity to pick up additional shifts. Extra shift pay if you elect to pick up additional shifts 1:1 patient ratio; see 4-7 patients per shift Competitive compensation with quarterly and annual bonuses Comprehensive benefits, including CME and license assistance Onsite training and growth opportunities Company vehicle, malpractice, and liability coverage What You’ll Do Perform comprehensive health assessments and diagnose mild to complex illnesses Provide therapeutic interventions including splinting, suturing, wound care, minor procedures, IV fluids, and medication administration Order, perform, and interpret point‑of‑care laboratory studies Analyze diagnostic data to guide treatment plans and ensure continuity of care Educate patients and families on condition management and recovery Navigate difficult conversations related to goals of care and end‑of‑life decisions Communicate effectively with patients, families, medical power of attorney, primary care providers, and care partners Accurately and thoroughly document all patient encounters Adhere to evidence‑based clinical standards, safety protocols, and performance metrics Attend required training sessions and clinical team meetings Drive a DispatchHealth‑branded vehicle to patient homes and deliver care independently Perform additional duties as assigned What You’ll Need Current, unrestricted NP or PA license in the state of practice National board certification (current) Minimum 2 years of APP experience in Emergency Medicine, Urgent Care, Internal Medicine, Hospital Medicine, Family Practice, Cardiac, Pulmonary, or related settings BLS and ACLS certification Prescriptive authority and active DEA (as required by state) Valid driver’s license with clean driving record. Continued employment is contingent upon maintaining an acceptable driving record in accordance with company policy and insurer requirements.
