LICENSED PRACTICAL NURSE (Ambulatory Care: Centralized Medication Refill )

Responsibilities*

• Provide direct and individualized nursing care to an assigned group of patients under the direction of a Registered Nurse and in accordance with established institutional and unit policies.
• Observe patient status; report and document changes, interventions, and nursing care provided.
• Provide information to assist in determining appropriate nursing care and discharge planning for assigned patients.
• Administer and document medications according to established standards.
• Initiate appropriate measures in emergency situations.
• Instruct patients and families according to a teaching plan established by a Registered Nurse.
• Process patient care orders using the electronic ordering system, as required in certain areas.
• Answer incoming calls and electronic messages related to medication refills.
• Verify patient identity and confirm prescription details.
• Document all refill requests and communications accurately in the electronic medical record (EMR/EHR).
• Utilize electronic protocol database to review and process routine refill requests following clinical protocols or standing orders
• Route refill requests requiring provider approval or consultation to the appropriate clinical staff.
• Collaborate with providers, nursing staff, pharmacy personnel, other disciplines, and community resources to resolve discrepancies, support patient needs, and ensure continuity of care.
• Communicate prescription status to patients and pharmacies.
• Participate in unit staff meetings focused on problem-solving, planning, and improving patient care.
• Assist in research projects and quality assurance activities.
• Provide oversite to assistive personnel, as appropriate.
• Perform other related duties as assigned.

• Maintain current knowledge of insurance regulations, medication formularies, refill guidelines, clinical protocols, and standing orders relevant to the role.
• Demonstrate adaptability and flexibility in response to changing patient, clinic, department, and institutional needs.
• Support program needs, including mentoring and orienting new staff, adapting to changes in technology and workflow, and responding to evolving organizational priorities as the unit grows.
• Demonstrate proficiency with typing and electronic health record documentation.
• Attend and actively participate in meetings related to this position. Meetings will be conducted in person or, when remote participation is required, staff are expected to be on camera and actively engaged.
• Demonstrate willingness to adapt to different patient populations, including pediatrics and adult medicine, based on patient care needs. Orientation and training will be provided for new subspecialties within primary care based on divisional expansion.
  
  
  

Supervision Received: General supervision is received from a Physician or Registered Nurse.

Physical Requirements: This position is primarily sedentary but may require extended periods of standing and walking. The role may also involve lifting, positioning, pushing, and/or transferring patients in accordance with established safety guidelines and procedures.

Required Qualifications*

• Graduation from an approved school for practical nursing.
• Current licensure as a Licensed Practical Nurse (LPN) in the State of Michigan.
• Successful completion of a pharmacology course in an accredited program.
• This classification requires Primary Source Verification
• Minimum of one (1) year of nursing experience in the past five (5) years in healthcare, pharmacy, or call center setting.
  
  
  

Desired Qualifications*

• Familiarity with EMR systems. (EPIC).
• Minimum of one year of ambulatory care experience.
• Experience working with UMHS Policies.
• LPN experience within an ambulatory care setting (within the past five (5) years).

Modes of Work

Union Affiliation

This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
