The Wound Care Nurse reports directly to the Director of Nursing (DON) or designee and works closely with nursing staff, CNAs, physicians, and the interdisciplinary team.

Key Responsibilities

  • Assess and monitor residents’ wounds, skin conditions, and overall wound-care needs.

  • Supervise and help ensure the highest quality of wound care is provided to our residents.

  • Complete wound and skin assessments for new admissions, readmissions, and residents experiencing changes in condition.

  • Review skin assessment documentation for accuracy and completeness.

  • Conduct regular rounds to ensure wound-care treatments and prevention programs are being followed.

  • Maintain accurate wound tracking records and submit updated weekly wound flow sheets to the Director of Nursing.

  • Review new admissions to ensure appropriate wound-care documentation and physician orders are completed and accurately transcribed.

  • Identify and report significant changes in residents’ skin or wound conditions in a timely manner.

  • Ensure care plans are updated when treatments or physician recommendations change.

  • Monitor CNA accountability records to ensure they accurately reflect residents’ current skin conditions.

  • Educate and support nursing staff on wound-care protocols, skin integrity, prevention strategies, and proper documentation.

  • Monitor infection-control practices related to wound care and nursing services.

  • Assess resident devices and equipment that may impact skin integrity and make appropriate recommendations.

  • Participate in Quality Assurance meetings as scheduled.

  • Collaborate with the interdisciplinary team to promote wound healing, prevent skin breakdown, and achieve positive resident outcomes.

  • Keep the Director of Nursing or designee informed of concerns, changes, and any corrective actions that may be needed.

  • Perform additional nursing and wound-care duties as assigned.

Qualifications

  • Current and active nursing license in Pennsylvania.

  • Wound care, skilled nursing, or long-term care experience preferred.

  • Knowledge of wound assessment, pressure injury prevention, treatment procedures, infection control, and skin integrity management.

  • Strong clinical assessment, communication, and documentation skills.

  • Ability to work independently while being an active and supportive member of the nursing team.

  • Wound care certification or specialized wound-care training is a plus!
