Minimum Qualifications

Education: High School Diploma or GED preferred.
Experience: Minimum six (6) months of related home health care, direct care, personal care, or human services experience preferred.
Certifications/Licenses: Current New York State Home Health Aide (HHA) certification required. Current CPR and First Aid certification preferred. Valid New York State Driver's License may be required for certain assignments.
Work Eligibility: Must be authorized to work in the U.S.
Work Location Requirement: Onsite and community-based services delivered in consumers' homes and community settings.

About the Role

The Home Health Aide (HHA) provides compassionate, person-centered care and support to individuals residing in their homes and community settings. This direct care position assists consumers with personal care, daily living activities, meal preparation, mobility, housekeeping, and community engagement while promoting independence, dignity, safety, and quality of life. Working under the supervision of a Registered Nurse or designated supervisor, the Home Health Aide helps maintain a safe and supportive environment and promptly reports any changes in a consumer's health or well-being.

Primary Responsibilities 

• Provide personal care assistance including bathing, grooming, dressing, toileting, skin care, and oral hygiene.
• Assist with activities of daily living, including eating, mobility, transfers, ambulation, and maintaining independence.
• Prepare and serve meals in accordance with dietary requirements and nutrition plans.
• Perform light housekeeping duties, including laundry, dishwashing, changing linens, and maintaining a safe living environment.
• Observe, document, and report changes in physical condition, mental status, behavior, or home environment.
• Follow individualized care plans and agency policies and procedures.
• Provide medication reminders and monitor medication compliance as directed.
• Escort and accompany consumers to medical appointments, community activities, and other approved destinations.
• Promote consumer dignity, choice, self-determination, and independence.
• Maintain accurate and timely service documentation and reports.
• Report incidents, injuries, abuse, neglect, safety concerns, and emergencies in accordance with agency policy.
• Assist consumers in maintaining a safe home environment by identifying and reporting potential hazards.
• Communicate effectively with consumers, families, caregivers, and interdisciplinary team members.
• Adhere to infection control procedures, universal precautions, and health and safety standards.
• Participate in required trainings, staff meetings, and supervisory sessions.
• Perform other duties as assigned.
