Home Infusion Registered Nurse (RN)

$50 / hour

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Longmont and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. 

 

Responsibilities: RN (Registered Nurse)

  • Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System.

  • Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.

  • Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.

  • Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.

  • Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.

  • Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.

  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.

  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.

  • Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.

  • Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.

  • Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (preferred)

  • Skilled in accessing Implanted Ports; Central Lines; PICC lines

  • Strong IV placement skills

  • Strong organizational and communication skills

  • Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)

Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status. 3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided. 4.
