Registered Nurse – Case Manager

CentralCare Incorporated is seeking a Registered Nurse Case Manager to support the Case Management Program at Joint Base San Antonio (JBSA).

Qualifications

Bachelor of Science (B.S.) in Nursing.
Graduate of a nursing program accredited by NLNAC or CCNE.
Current, full, active, and unrestricted Registered Nurse license.
Minimum of 2 years of full-time RN experience in utilization management/utilization review or broad-based clinical nursing and clinical case management within the last 3 years.
Must have completed 30 hours of continuing nursing education within the last 3 years.

Key Responsibilities

Participate in all phases of the Case Management Program and ensure established standards of care are met.
Provide nursing expertise in assessment, planning, implementation, coordination, and monitoring.
Coordinate inpatient, outpatient, onsite, and telephonic case management.
Integrate case management and utilization management with nursing and social work services.
Develop and implement patient treatment plans to promote continuity of care and optimal wellness.
Coordinate care with healthcare professionals, community agencies, and specialty-care providers.
Accurately collect and document patient-care data.
Provide healthcare education to patients and caregivers.
Facilitate appropriate follow-up after discharge.
Recognize adverse signs and symptoms and respond appropriately to emergency situations.
Conduct research supporting improved clinical practice and patient outcomes.

Schedule
Monday–Friday. Work shifts will be between 7:00 AM and 4:30 PM. Normal schedules are 9-hour shifts, including a 1-hour unpaid lunch, not to exceed 40 hours per week or 80 hours every two weeks.
