About the Role

As a Home Health Aide, you will provide high-quality, personalized care to patients in their homes. Your duties will include:

• Ensuring quality and safe delivery of home health care services
• Implementing patient care plans in accordance with agency policies and procedures
• Participating in patient case conferences and reporting updates to the Registered Nurse
• Assisting patients with daily living activities, such as bathing, dressing, and medication management
• Accompanying patients to doctor's offices or hospital visits
• Providing emotional support and companionship to patients and their families

Qualifications

To be considered for this role, you must:

• Hold a High School Diploma or equivalent
• Be certified as a Home Health Aide
• Have at least 1 year of Home Health Care experience (experienced preferred)
• Be able to read, understand, and execute instructions
• Be able to communicate and write effectively
• Be able to work and deal within a stressful environment

Key Responsibilities Escort residents safely to and from clinic and dialysis appointments. Arrive one hour prior to scheduled clinic appointments.
