Department Overview

Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). The patient population includes pregnant people, babies, children, adolescents, adults, and geriatrics. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include, but are not limited to, oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management.

Required Qualifications

  • BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
  • ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
  • Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS from AHA required. New hires will be enrolled and required to complete during orientation.
  • Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

  • Current clinical nurse experience in an ambulatory care setting including telephone triage.
  • Experience working in Primary Care, a Rural Health Center (RHC) or a Federally Qualified Health Center (FQHC) setting within the last three years.
  • Nursing professional certification in Ambulatory Nursing or other relevant specialty-population.

Qualifications Experience / Requirements / Qualifications Holds a current, unrestricted license as a Registered Nurse within the State Must possess an active BLS/CPR Certification (AHA preferred) Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services. Ability to maintain confidentiality of all proprietary and/or confidential information Excellent communication, organizational, and problem-solving skills Able to work independently and in a team Flexible schedule and availability to work overtime when needed Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
