Work Schedule:  

  • Shift times vary by month and are split with CO-APP, mostly days, holidays & weekends rotate with group  
  • Approximately 15 shifts per month

Job Description Summary 
 

We are seeking an experienced inpatient APP to join our Hospitalist team in Cedar City, UT. Cedar City Hospital is a 48-bed facility that offers a broad range of wellness, diagnostic, and treatment services, features a Level III Trauma Center, and is supported by Intermountain’s virtual hospital and Life Flight air medical transport.

The Hospitalist Advanced Practice Provider (APP) plays a critical role in the management of adult and pediatric hospitalized patients, with a primary focus on those on general medicine floors. In this role you will work with a multidisciplinary team of doctors, case managers, nurses, dietitians, and pharmacists to provide optimal patient care and outcomes. 

 
Key Responsibilities: 

 
Advanced Practice Providers (APPs) are accountable for the provision of patient care at the top of licensure, within their scope of practice. Licensed, credentialed and privileged to perform such duties as: 

  • Conduct comprehensive assessments, including history and physical examinations. 
  • Develop, implement, and evaluate treatment plans in collaboration with attending physicians. 
  • Manage acute and chronic medical conditions, including initiating appropriate diagnostic and therapeutic interventions.  
  • Work closely with multidisciplinary teams, including physicians, nurses, respiratory therapists, and pharmacists, to ensure coordinated patient care. 
  • Facilitate communication between patients, families, and healthcare teams, ensuring that patients and their families are informed and involved in care decisions and care transitions. 
  • Maintain accurate and timely documentation of patient encounters, treatment plans, and progress notes in the electronic health record (EHR) system. 
  • Ensure compliance with hospital policies, regulatory requirements, and best practice standards. 
  • Participate in ongoing professional development and continuing education to stay current with advancements in critical care and internal medicine. 
  • Engage in quality improvement initiatives within the hospitalist program and hospital. 
  • Participate in hospital committees, task forces, and meetings as required.

Qualifications 

​Licensure/Certification: 

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in Utah 
  • Board certification as an Acute Care Nurse Practitioner (ACNP), Family Medicine Nurse Practitioner (FNP) or Physician Assistant (PA-C) 
  • DEA Certificate II through V - DOJ Drug Enforcement Division   
  • ACLS and BLS certification for healthcare providers 
  • PALS Certification for healthcare providers 

Experience: 

  • Minimum of 2 years experience working as an inpatient APP preferred 

Skills and Abilities: 

  • Strong clinical decision-making and critical thinking skills. 
  • Ability to work independently and as part of a team in a fast-paced, high-acuity environment. 
  • Excellent communication, interpersonal, and organizational skills.

  • Ability to work in a hospital environment, which may involve exposure to infectious diseases and high-stress situations. 
  • Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.    
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.   
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.     
  • May be expected to stand in a stationary position for an extended period of time.

Job Summary: ***NOW OFFERING A $4,000 SIGN ON BONUS*** Hours: Full-time; 40 hours per week Union: None Union Name: None Patient Facing: Yes Boston Health Care for the Homeless Program is seeking a dynamic, patient-centered, proactive, and energetic Nurse Practitioner/Physician Assistant to join a multidisciplinary team. This position will provide comprehensive, high-quality primary, episodic and/or recuperative care at our main campus and surrounding shelter-based clinics. The ideal candidate for this role must be a self-starter and have the ability to lead a care team in partnership with other clinical peers. Our practice is collaborative, supportive and innovative. We are looking for someone who shares these qualities. Responsibilities: Provides comprehensive, high quality episodic and/or recuperative care at various clinical sites staffed by Boston Health Care for the Homeless Program. May work at shelter-based clinic, hospital-based clinic, respite facility or on street-based outreach based on program needs. Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program. Refer patients as appropriate to medical specialists, hospital admission, recuperative care, and detox facilities, providing appropriate information for continuity of care. Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts. Provide education to patients on health promotion and disease treatment and prevention. Work as part of multi-disciplinary team (medical, nursing, dental, BH, pharmacy, case management) in caring for patients. Administrative Responsibilities: Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Maintain active DPH, DEA, and professional license, managed care credentialing and CPR training. Some on-call and holiday coverage required. Qualifications: Licensed by the Commonwealth of Massachusetts. Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency. At least one year of direct NP/PA experience. Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to start date, applicant must submit certification within 90 days of start date. Experience in working with underserved populations, mentally ill, homeless and/or persons with substance use disorders preferred. Bilingual proficiency preferred.
