An Average Day: 
(Includes, but not limited to)

  • Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
  • Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique
  • Provide appropriate pain/symptom management
  • Evaluate patients response to treatments/medications
  • Participate in personal, professional growth and development
  • Participate in organization's quality assessment performance improvement teams and activities
  • Perform other duties as assigned by the registered nurse
  • All other duties as assigned 

 

Requirements: 

  • Must currently be licensed as an LPN/LVN in the State 
  • Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
  • Must understand the philosophy of hospice concept and understand needs of the terminally ill
  • Must be flexible in work hours
  • Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Case Manager LVN will cover two facilities. $35-38/hr POSITION SUMMARY The purpose of your job position is to coordinate delivery of services to managed care and Medicare residents in accordance with current federal, state and local standards and regulations that govern the facility and in collaboration with the Administrator, Director of Marketing, and other facility staff. The Case Manager monitors and documents the cost effectiveness of treatment provided, coordinates the admission and discharge process, serves as the resident/family advocate, and acts as liaison to insurance and medical management professionals. They will treat each resident fairly, kindly, and show them dignity and respect. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult. They will relate all pertinent information concerning a resident’s condition to a charge nurse when required. They will be committed to always doing the right thing.
