Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life. The Rely center is "A Safe Place for Help" which includes a Psychiatric Urgent Care for all ages and an adult 23 hour stabilization unit.

Position Summary

This position will provide quality integrated psychiatric health care services and medication management to adults in an outpatient setting, and will perform as an independent and interdependent member of the medical services team providing integrated general medical and behavioral health care to clients in need of services. The Rely Center team is for individuals who have a passion for and are comfortable working in fast-paced environments, assisting individuals in mental health and/or substance use crises, and collaborating with a multidisciplinary team.  This is a full-time position, with potential schedule adjustment/options in the future. Must have reliable transportation to travel to other sites for coverage, trainings and/or meetings

 

  • Provide comprehensive and integrated health care services to children, adolescents, adults, and elderly patients in need of services
  • Perform telehealth visits when appropriate for medical patients
  • Advanced Practice Services including:
    • Complete psychiatric evaluations/assessments
    • Integrated medical evaluation, assessment and care planning
    • Refer client to primary care physician and/or specialist, as needed
    • Coordinate psychiatrist evaluations
    • Order appropriate laboratory tests, review results and make necessary adjustments
    • Perform and interpret basic screens
    • Prescribe and/or monitor medications according to appropriate psychiatric diagnosis, symptoms and practice guidelines
    • Provide medication monitoring/management in individual and/or group setting

Licensure

  • Must be recognized as a Registered Nurse (RN) with Prescriptive Authority by the State of Indiana
  • Must have current Drug Enforcement Administration (DEA) certificate and State of Indiana Controlled Substance Registration (CSR)
  • Must have PMHNP certification 
  • Must have current BLS certification

Education/Experience

  • Master’s degree in Nursing or a Doctorate degree in Nursing from a college/university accredited by the US Department of Education required
  • One (1) year supervised experience with Severely Mentally Ill (SMI) population preferred

JOB DUTIES: Provides excellent medical care with the ARS philosophy outlined by the Chief Medical Officer and Medical Director. Screen for co-occurring Primary Mental Health issues. Screen for Substance Use Disorders (SUD) including psychoactive substance toxicity, intoxication, and withdrawal symptoms. Must have knowledge of medications, assessment for medication complications, detoxification protocols and prescribing medications in general Screen for danger to self or others with an evidenced based assessment tool—the Columbia Suicide Screening Rating Scale (CSSRS) and the Patient Health Questionnaire-9 (PHQ-9). Analyze and interpret data to determine treatment recommendations and priorities. Screen for nutrition deficiencies and make interventions or refer for a nutrition consultation as appropriate. Screen for pain and understands treatment strategies for pain including pharmacologic and nonpharmacologic approaches. Strategies should reflect an individualized approach based on the individual’s current presentation, medical judgment and the risks and benefits associated with strategies, including potential risk of dependency, addiction, and abuse.
