River Falls Senior Living is seeking a compassionate and motivated Licensed Practical Nurse (LPN) to join our resident care team.

This is a full-time, Monday-Friday day-shift position with a flexible schedule. The LPN plays an important role in providing quality, person-centered care and coordinating services for residents in our assisted living community.

What You'll Do

  • Assess resident needs and help develop and implement Individual Service Plans (ISPs).

  • Monitor residents' health status and communicate changes to the appropriate healthcare providers.

  • Coordinate care with residents, families, physicians, VNA/home health agencies, PT, OT, and other service providers.

  • Provide guidance and support to Resident Care Assistants and Resident Care Supervisors.

  • Assist with evaluating prospective residents as part of the admission process.

  • Identify safety concerns and initiate appropriate corrective action.

  • Serve as a liaison between residents, families, care staff, administration, and healthcare providers.

  • Promote quality, person-centered care while supporting residents' independence and dignity.

  • Perform other related duties as assigned by the Wellness Director.


What We're Looking For

  • Current Massachusetts Licensed Practical Nurse (LPN) license in good standing.

  • Previous experience in assisted living, long-term care, home health, or a related healthcare setting preferred.

  • Strong communication and organizational skills.

  • Ability to work effectively as part of an interdisciplinary care team.

  • Compassionate and resident-focused approach.

  • Ability to recognize changes in resident condition and communicate concerns appropriately.
