Description

Responsible for the care activities and goal achievements of assigned patients by assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient's response to care. Responsible for health teaching and discharge planning. Acts as a member of a designated care team and collaborates, coordinates and guides team members in planning patient care activities. Demonstrates behaviors, which reflect the scientific knowledge, judgment and teaching skills of a professional nurse


Responsibilities

1. Conducts Nursing assessment based on interview, examination, observation and review of records of the patient

2. Ensures that the patients progress, or lack of progress, is reflected in progress notes and in the revised care plans

3. Discharge planning consistent with the patient's status and reflects inter-departmental and inter-agency communication

4. Communicates information to the patient based on the patient's level of awareness and readiness to learn

5. Supports orientation of new staff members by observing professional behaviors, providing feedback to the staff member and by acting as a preceptor, role model and a resource person

6. Functions as a professional role model and mentor for students and volunteers.

7. Patient care assignments reflecting an awareness of patient needs and skill level of personnel

8. Organization of provision of care to meets priority needs of patients and unit

9. Communicates condition of patients and unit requiring additional intervention to the leadership person in the relevant discipline

10. Participates in the development of other staff members

11. Communicates with peers in an appraisal of practice as it relates to self and other nursing personnel

12. Reflects a collaborative effort in meeting the needs of the patient and unit through communication and nursing actions

13. Provides direction, assistance and support

14. Assumes accountability and responsibility for completion and quality of assigned tasks

15. Identifies and communicates to immediate supervisor the need for further staff education

16. Reflects awareness of authorities and responsibilities of nursing management levels by communication with nursing leadership

17. Seeks and utilizes nursing leaders as role models and resource persons

18. Reflects collaborative effort in meeting patient and unit needs by communication with other disciplines

19. Reflects the philosophy, goals and objectives of the Department of Nursing and the Hospital Center in communication and interaction with families and other groups


Qualifications
  • Bachelor of Science with a major in nursing preferred.  Associate's degree in nursing is considered only with relevant experience and/or active matriculation in a BSN Program if approved by Chief Nursing Officer or designee
  • 2 Years main OR pre-op and post-op, or 2 years telemetry or PACU experience required
  • Licensed as a registered nurse with current registration in New York State
  • BCLS, ACLS and PALS from AHA required 
  • CAPA preferred

Job Description The Outpatient Radiology Nurse provides safe, efficient, and compassionate nursing care to patients undergoing outpatient diagnostic and interventional imaging procedures. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Principal Accountabilities Patient Assessment: Perform systematic, comprehensive, and accurate assessments. Assess potential discharge needs during the initial assessment process. Administers medications to patients and monitor effects of medications and treatments and report adverse effects in a timely manner. Assess patient for pain level. Provide appropriate intervention, reassessment, and documentation. Systematically monitor the maintenance of protocols, standards, and plans of care. Plans and provides individualized patient education. Clinical Skills: Perform nursing-related skills and activities competently. Analyze situations critically. Maintain complete and accurate documentation. Identify and document realistic long and short-term goals. Complete interdisciplinary plan of care record. Participate in interdisciplinary rounds. Outreach and Education: Participate in local, state, and national public health planning initiatives. Assist with monitoring and evaluation of grant-funded projects. Provide education to communities, patients, and families in accordance with grant requirements. Attend external meetings and field-based outreach events. Performs other related duties as assigned. Job Description Registered Nurse (RN) – Surgical Oncology Physician Practice Position Summary The Registered Nurse (RN) is a vital member of the multidisciplinary surgical oncology team, providing comprehensive nursing care to patients throughout the continuum of surgical cancer treatment. The RN collaborates closely with surgeons, advanced practice providers, medical assistants, and ancillary staff to coordinate patient care, provide education, facilitate treatment planning, and ensure exceptional clinical outcomes. This role requires strong clinical judgment, excellent communication skills, compassion, and the ability to manage complex patient needs in a fast-paced outpatient setting. Essential Duties And Responsibilities Clinical Care Perform comprehensive nursing assessments and triage patients in person and by telephone. Evaluate patient symptoms and determine appropriate interventions or escalation of care. Monitor patients before and after office-based procedures. Review laboratory, pathology, and imaging results, communicating findings and provider recommendations to patients. Coordinate preoperative and postoperative care for surgical oncology patients. Identify urgent clinical concerns and facilitate timely evaluation and treatment. Care Coordination Coordinate referrals to specialists, medical oncology, radiation oncology, genetics, nutrition, social work, and other support services. Schedule diagnostic testing and ensure timely completion. Ensure continuity of care across outpatient, inpatient, and procedural settings. Clinical Documentation Document nursing assessments, interventions, patient education, and communications accurately in the electronic medical record (EMR). Maintain accurate medication reconciliation and allergy documentation. Process medication refills according to practice protocols. Complete disability forms, FMLA paperwork, and other clinical documentation as appropriate. Telephone Triage Assess patient concerns using evidence-based triage guidelines. Provide clinical advice within scope of practice. Escalate urgent or emergent concerns to the provider. Document all patient communications in the EMR. Quality and Compliance Maintain compliance with HIPAA, OSHA, CMS, and organizational policies. Participate in quality improvement initiatives.
