How you’ll make an impact in this role
- Coordinate comprehensive care transitions by serving as the primary link between all clinical disciplines, ensuring every patient receives a seamless and well-integrated recovery experience.
- Design individualized discharge pathways that integrate patient values and family preferences, ensuring a safe and sustainable transition from the hospital to the next stage of care.
- Advocate for vital patient resources by identifying and removing systemic barriers to care, ensuring every individual has the medical necessity and support required for their specific diagnosis.
- Evaluate clinical documentation accuracy to provide expert recommendations on levels of care, ensuring that medical records reflect the true complexity of the patient's journey and treatment options.
- Drive continuous care improvement through active collaboration with the care transition team, constantly re-evaluating health plans to adapt to a patientâs changing condition in real-time.
What minimum requirements you’ll need
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job
- transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Additional Preferences:
- Case Management Expertise: A minimum of 3 years of dedicated experience in a Case Management role within a healthcare setting.
- Discharge Planning Expertise: Demonstrated experience in coordinating complex patient transitions and multidisciplinary care plans.
- Schedule: Monday-Friday 8:00AM-4:30PM | Some Weekends and Holiday rotation.
