About the Role:

As an Evening Shift LPN at Terraces of Lake Worth Care Center, you will:

Provide direct patient care to our residents, including administration of medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Maintain accurate and thorough records of patient care and progress
Collaborate with other healthcare professionals to ensure high-quality patient care and outcomes

Job Requirements:

Current LPN licensure in the state of Florida
High school diploma or equivalent required; Associates degree or Bachelor's degree in Nursing or related field preferred
Minimum 1 year of experience in long-term care, sub-acute care, or rehabilitation settings; experience with geriatric population preferred
