Job Summary:

Unity Health and Rehabilitation Center, Setting standards in Rehabilitation and Skilled Nursing Care, is seeking a dedicated, compassionate and experienced MDS RN Coordinator to join our team. As a vital member of our care delivery team, you will be responsible for ensuring the highest quality of care for our long-term care patients.

About the Role:

As an MDS RN Coordinator, you will be responsible for:

Collecting and analyzing data to develop a comprehensive care plan for each patient
Coordinating with care providers, therapists, and other healthcare professionals to ensure seamless care delivery
Assisting with the development and implementation of individualized treatment plans
Monitoring patient progress and making adjustments to care plans as necessary
Maintaining accurate and timely records of patient care and treatment

Responsibilities:

Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
Gather and analyze data to identify trends and areas for improvement in patient care

Requirements:

Current certification as aRegistered Nurse (RN) in the state of Florida
Minimum of 2 years experience in a long-term care setting, as an MDS RN Coordinator
Strong analytical and problem-solving skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Familiarity with MDS software and electronic medical records
