Office Location: Palatka   Coverage area: Palatka (can support other East Coast Branches also)    

Schedule: PRN After hours on call. Must work a minimum of 2 shifts per biweekly pay period.

As a On-Call RN You will:    

  • (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis  
  • (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis 
  • Assess and manage urgent symptoms quickly and compassionately 
  • Administer medication or adjust care based on hospice protocols or provider orders 
  • Communicate with the physician regarding changes in condition and obtain orders as necessary 
  • Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps 
  • If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team  
  • Complete timely documentation and communication

Qualifications

  • Graduate of an accredited nursing school with current licensure to practice in state of operation 
  • One year nursing experience required, 3+ years preferred 
  • Experience in hospice or a similar setting preferred 
  • Knowledge of the hospice philosophy of care 
  • Commitment to clinical and documentation excellence 
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order 
  • Possess and maintain current CPR certification in states where required.

Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. Assignments are performed appropriately to age and developmental level of patients being served. Qualifications Age of Patients Served: All Age Groups. Required Education:: Associate's Degree Specific course work: Graduate of an accredited school of nursing. New hires effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position. Overview Overview The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring patients’ health through the nursing process.
