Job Summary:

We're seeking a compassionate and skilled Registered Nurse to join our team at Concord Care of Hartford in Fowler, Ohio. As a valued member of our healthcare team, you'll provide direct patient care, collaborate with other healthcare professionals, and participate in quality improvement initiatives to enhance patient outcomes and staff efficiency.

Responsibilities:

• Provide direct patient care, including assessing, planning, implementing, and evaluating individualized care plans
• Administer medications, treatments, and therapies as ordered by physicians
• Collaborate with other healthcare professionals to develop and implement comprehensive care plans
• Monitor and report patient status, progress, and changes to nursing staff and medical team
