Nurse Practitioner (APRN) – Home Health Support

The Lakes Home Care is seeking a licensed Nurse Practitioner (APRN) to support our home health operations. The NP will play a key role in ensuring compliance with Medicare guidelines, assisting with Plan of Care (POC) documentation, and conducting Face-to-Face patient encounters.

Responsibilities:

• Review and sign Plan of Care (POC) and recertifications

• Perform Face-to-Face encounters as required

• Support clinical documentation compliance (Medicare/AHCA)

• Collaborate with RNs, MDs, and administrative staff

• Assist in reducing hospital readmissions through patient follow-up

Requirements:

• Active Florida APRN license

• Active NPI number

• Medicare enrollment (PECOS) preferred

• Experience in Home Health or geriatrics preferred

• Knowledge of Medicare guidelines and documentation

Compensation:

• Per visit: (depending on service)

• POC review/signature: negotiable

• Monthly contract option available

PRN Nurse Practitioner – Telehealth (Home Health)

Position Summary The PRN Nurse Practitioner provides services including assessments, clinical evaluations, medication management, patient education, and collaboration with the interdisciplinary team. This role supports home health patients with chronic conditions, acute changes, post-hospitalization needs, and ongoing follow-up care. The NP will serve as a virtual and in person clinical resource, ensuring timely and appropriate care while assisting in preventing unnecessary ER visits and hospital readmissions. This is an as-needed position to provide flexibility and coverage for patient needs, high census periods, and staff PTO. Key Responsibilities Conduct virtual patient assessments (audio/video) to evaluate symptoms, chronic disease status, medication needs, and overall well-being. Provide timely clinical recommendations, interventions, and treatment plans aligned with home health goals. Support transitions of care by completing post-hospital and post-procedure follow-up visits. Provide education on medication adherence, disease management, and self-monitoring strategies. Empower patients to manage their health between in-person nursing visits.
