This field-based role focuses on providing essential personal care and emotional support to terminally ill patients, ensuring comfort, dignity, and quality of life across the San Gabriel Valley (SGV) region. The ideal candidate will work independently in the field, confidently manage digital documentation, and possess a deep respect for diverse cultural backgrounds. Provide skin care to prevent breakdown, assist with safe ambulation, transfers, and prescribed exercises, ensuring a safe and supportive environment. Monitor and record vital signs as directed. Document and immediately report any changes in the patient’s physical, mental, or emotional condition to the Case Manager or Supervisor. Use the HospiceMD EMR platform to log patient visits, care plans, and clinical narratives accurately and timely, in accordance with agency policy and regulatory standards. Travel reliably between assigned patient locations across the SGV territory. Track business travel and accurately submit monthly mileage invoices using Concur for reimbursement. Fluent verbal and written English communication is required. Strong computer knowledge with previous hands-on experience using Electronic Medical Records (EMR) software, specifically HospiceMD, and comfort with digital expense tools like Concur. Valid driver’s license, a reliable and well-maintained vehicle, and active auto liability insurance to meet mandatory field travel requirements. Deep empathy for the ill and elderly, a calm demeanor around end-of-life care, high integrity, and strict adherence to HIPAA patient confidentiality regulations.
