Southwest Palliative Plus, a leading provider of compassionate hospice care, is seeking a skilled and dedicated Spanish-speaking Hospice RN PRN to join our team in Dallas, TX. As a Hospice RN with our organization, you will have the opportunity to make a meaningful impact on patients' lives and support their families while providing high-quality end-of-life care.


Job Summary

As a Hospice RN, your primary function is to administer compassionate nursing care to patients in their place of residence. You will take the lead in coordinating care with our Interdisciplinary Group (IDT), patients, families, and referring agencies to ensure the highest quality end-of-life support.


Key Responsibilities

• Fluent in Spanish
• Perform comprehensive initial and ongoing assessments of patients' physical, functional, and psychosocial needs
• Develop, implement, and document individualized plans of care to ensure quality and continuity of care
• Educate patients and their families regarding the disease process, self-care techniques, and end-of-life care
• Provide clinical direction and supervision to Hospice Aides and LVNs
• Maintain accurate, timely, and complete clinical documentation


Qualifications

• Current state license as a Registered Nurse (RN)
• Clinical care setting experience, with hospice experience preferred
• Valid driver's license, reliable transportation, and valid auto liability insurance
• Ability to lift up to 50 lbs and safely move patients
