Provides skilled nursing care to hospice clients in their homes or community health settings ensuring the delivery of outcome based, high quality, cost-effective care.

·       Reviews referrals, and initiates contact with client and family.

·       Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status.  Demonstrates clinical expertise in symptom management and end of life issues.

·       Develops a plan of care in collaboration with client and all disciplines based on assessment data which documents need for services, appropriate discharge planning, short- and long-term goals, and the utilizations of appropriate resources.  Facilitates peaceful death and assures appropriate bereavement follow up through timely documentation of bereavement assessment.  Modifies plan of care as conditions change.

·       Demonstrates understanding of payer information in order to maximize care planning.  Utilizes Hospice Levels of care appropriately.

·       Follows IDT process of approving non-formulary medications, treatments, and DME.

·       Initiates and maintains a client record utilizing the Agency’s standard for documentation.

·       Makes follow-up visits to provide skilled care to assess disease progress and evaluates response to treatment so as to promote peaceful death; provides support to client/family utilizing teaching principles to promote and restore health and prevent disease and disability, as possible; coordinates care with providers.  Utilizes interdisciplinary team members to support patient/family needs.

·       Teaches patients and reinforces client/caregiver teaching and education.  Prepares/teaches/supports family regarding signs and symptoms of death, as appropriate.

·       Refers, collaborates, and coordinates all client care services.  Consults with physician regarding client needs and plan of care.

·       Directs LPNs in carrying out the established plan of care.  Directs Home Care Aides and supervises plan of care.


Qualifications


  • Minimum Requirements:

  • Education: Graduate of an Associate’s of diploma program in nursing

  • Experience: One year nursing experience OR one year nursing experience in a specialty

  • Licensure: License to practice in the State of Connecticut

  • Preferred:

  • Education: Graduate of NLN approved BSN program

  • Experience: Medical/surgery or home care experience

Does comprehensive assessment of resident through medical record review, appropriate interviews, and assessment of resident. 3. Completes MDS with appropriate RAPS, resident summary, and develops resident care plans. 4. Ensures timely completion of all aspects of the RAI process and timely transmission per State and Federal regulations. 5. As applicable, lead case management for Managed Care. 6. As directed by DNS/Administrator, participate in CMI meetings and billing meetings. 7. Communicate PDPM recommendations as appropriate. 8. Reviews Final validation report from State and makes corrections as needed. 9. Participates in the weekly Medicare Meeting and communicates pertinent MDS information. 10. Reviews each Medicare resident for appropriateness of Medicare coverage. 11. Obtains monthly Quality Indicator Reports and provides them to the DNS/ADNS. 12. Performs other duties as directed by the Director of Nursing Services. 13. Assure timely completion of all aspects of resident assessment process and timely transmission for federal and state agencies. 14. Review and maintains final validation report from state and corrects as needed. 15. Attends meetings as assigned by the DNS/ADNS

Resource in all areas of clinical care and resolves work related complaints of charge nurses and CNA's during assigned shift. 4. Makes rounds on nursing units to ensure maximum care and to ascertain condition of residents. Communicates family concerns. 5. Responds to emergencies within the facility and provides medical oversight. 6. Verifies that all clinical changes are reported to the physician. Verifies physicians’ orders as needed. 7. Secures pre-admission report on new residents. 8. Ensures a safe environment for residents and staff. 9. Ensures all documentation on medical record is complete and is accurate (including A & I’s; 24 Hour Nursing Report; Nursing Assessments; MD Orders; documentation involving Admissions, discharges, deaths, and Transfers). Conducts chart audits. 10. Ensures availability of supplies and equipment. Reports malfunctioning equipment to DNS/ADNS. 11. Follows up with the physician and medical director as necessary to ensure compliance with scheduled visits. 12. Assumes supervisor responsibility for the facility on 2nd and/or 3rd shifts and/or weekends. RN Supervisor or Charge Nurse. 13. Audits emergency drug cart and crash cart to assure they are properly stocked. 14. Audits control of drugs and counts controlled medications at the beginning and end of each shift. 15. Ensures that all peripheral IVs are initiated per facility policy and monitors overall IV program. Ensures IV policy is updated annually. 16. Responsible to assign nursing personnel in emergencies to maintain patient care hours. 17. Assists in the recruitment, selection and of orientation nursing staff as needed. 18. If facility directed, completes 90-day and annual employee performance reviews. 19. Logs and returns all eligible medications to the pharmacy in a timely manner. 20. If facility directed, assumes responsibility for Medicare residents by attending weekly Medicare meetings and assuring that certifications are completed. 21. Attends meetings appropriately including but not limited to Fall Risk, Restraint, Wound Care, and Weight Loss meetings. 22. Assumes any other duties as assigned by DNS/ADNS. Performs other related duties and attends meetings as may be deemed necessary by Supervisor / Administrator and or Director
