Primary Responsibilities: 

  • Assess, plan, and implement person-centered strategies
  • Connect members with financial, psychosocial, community, and state resources
  • Coordinate care across the continuum and serve as a key point of contact

Required Qualifications

  • Current unrestricted Texas RN license
  • 2+ years of experience in a community health or healthcare setting
  • Intermediate level of proficiency with Microsoft Office applications
  • Reliable transportation, a valid driver, and ability to travel within assigned territory

Preferred Qualifications

  • Experience with Medicaid, LTSS, or long-term care services
  • Knowledge of HCBS and integrated care principles
  • Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
  • Bilingual in English and Spanish
