Responsibilities:

  • Deliver high-quality patient care to residents, including administering medications, performing treatments, and providing personal care

  • Conduct initial and ongoing assessments to identify resident needs and develop individualized care plans

  • Monitor and report changes in resident condition to the healthcare team and physicians

  • Collaborate with other healthcare professionals to ensure comprehensive resident care

  • Maintain accurate and detailed records of resident care and treatment

  • Demonstrate a commitment to safety, quality, and patient satisfaction

  • Support and promote a positive, supportive, and inclusive work environment

Requirements:

  • Minimum of 0 years of experience as a licensed practical nurse, preferably in long-term care or similar setting

  • Current LPN license in the state of Ohio

  • Ability to work varying shifts, including night shifts, weekends, and holidays

  • Strong communication and interpersonal skills

  • Ability to lift, transfer, and move residents as needed

  • Familiarity with electronic medical records and medication management systems
