Are you an experienced hospice RN who loves case management but would rather spend less time driving and more time caring for patients?

Serenity Hospice PA is seeking an experienced Hospice RN Case Manager for a dedicated facility-based position in West Chester, Pennsylvania.

This is a unique opportunity to case manage hospice patients while being assigned to just one facility. Instead of driving throughout a large territory or traveling between multiple facilities each day, you will report to one West Chester location, allowing you to spend more of your day focused on patients, families, and clinical care.

You will still have the responsibilities of a true Hospice RN Case Manager, you'll manage your patients, coordinate their care, assess changing needs, communicate with families and facility staff, and collaborate with the hospice interdisciplinary team. The difference is simple:

ABOUT THE ROLE

As the dedicated Hospice RN Case Manager for this West Chester facility, you will serve as Serenity Hospice's primary clinical presence at the location and oversee the hospice care of patients residing there.

This is not an inpatient hospice unit. Patients reside within the facility and receive hospice services there.

You will build strong relationships with patients, families, facility nurses, administrators, caregivers, physicians, and the hospice interdisciplinary team. The ideal candidate will become a trusted clinical leader and resource within the facility.

YOUR RESPONSIBILITIES

  • Manage and coordinate individualized hospice plans of care for patients residing at the facility

  • Perform comprehensive nursing assessments, routine visits, and ongoing evaluations

  • Monitor changes in patient condition and provide timely clinical intervention and symptom management

  • Provide compassionate nursing care focused on comfort, dignity, and quality of life

  • Educate and support patients and families regarding disease progression, medications, hospice philosophy, and end-of-life care

  • Build strong, trusting relationships with patients and families through consistent communication and compassionate care

  • Communicate and collaborate with facility nurses, administrators, caregivers, physicians, and the hospice interdisciplinary team

  • Serve as a clinical leader and trusted resource for facility staff

  • Educate facility staff regarding hospice philosophy, symptom management, and end-of-life care

  • Coordinate medications, equipment, supplies, and other hospice services

  • Participate in interdisciplinary team meetings and communicate changes to patient plans of care

  • Maintain accurate, timely, and compliant clinical documentation

  • Advocate for patients and families and ensure their goals and comfort remain central to the plan of care

  • Foster a strong, professional partnership between Serenity Hospice and the facility

  • Current, unrestricted Pennsylvania RN license

  • Strong hospice nursing experience

Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
