POSITION OVERVIEW

The Licensed Practical Nurse (LPN) in our Memory Care Neighborhood provides compassionate, person-centered care for residents living with dementia and memory-related conditions. Working collaboratively with the care team, the LPN conducts assessments, administers medications and treatments, monitors resident health, coordinates care, and supervises caregivers. This role supports individualized care plans and uses person-centered dementia care practices to promote resident dignity, comfort, engagement, and quality of life.


Key Responsibilities

  • Provide person-centered dementia care, adapting care and communication to support resident dignity, comfort, independence, and quality of life.
  • Deliver resident-centered nursing care in accordance with individualized care plans, physician orders, and nursing best practices.
  • Conduct nursing assessments, monitor changes in condition, and communicate updates to families, physicians, and the care team.
  • Administer medications and treatments safely, monitor resident responses, and maintain accurate clinical documentation.
  • Develop and update individualized care plans based on resident needs, preferences, and health outcomes.
  • Collaborate with families, physicians, therapists, and interdisciplinary team members to ensure coordinated, high-quality memory care.
  • Provide clinical guidance and supervision to nursing assistants and caregivers, promoting excellence in dementia care and resident safety.
  • Advocate for resident rights, choice, dignity, and independence, creating a safe and engaging environment for individuals living with dementia.

Qualifications  

  • Active and unencumbered LPN/LVN license in the state of practice required.
  • Active CPR certification required.
  • Previous experience in long-term care, skilled nursing, rehabilitation, memory care, or geriatric nursing preferred.
  • Must be willing and able to complete the Trilogy Legacy Dementia Education Program during orientation and annually thereafter, as well as maintain competency validation.
