Responsibilities

The Home Health Field RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

  • Oversee the activities of the multi-disciplinary team that provides care to the patient
  • Monitor the provision of therapeutic services through written and verbal communication with all disciplines
  • Ensure that all visits scheduled and provided are authorized and covered by physician orders
  • Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
  • Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor
