Responsibilities:

  • Provide high-quality nursing care to patients with complex needs, both short-term and long-term, in a subacute and long-term care setting

  • Collaborate with the interdisciplinary team to develop and implement individualized care plans

  • Conduct patient assessments, administer medications, and perform treatments as ordered

  • Monitor patient progress, report changes, and adjust care plans as necessary

  • Foster a culture of patient-centered care, respect, and compassion among patients, families, and colleagues

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
