HouseCalls is an innovative, performance-driven program that brings care directly to members' homes. As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct annual in-home health assessments for Medicare Advantage and other plan members. This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers (PCP).

 

Primary Responsibilities:

  • Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based practice screenings including point of care testing (as appropriate)
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
  • Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate members on topics such as disease process, medication, and compliance.
  • Travel up to 85%

Required Qualifications:

  • Active NP or PA license or ability to obtain by start date.  Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP or PA license in state of assignment by start date;  OR for PAs - Physician Assistant national certification through NCCPA
  • Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand and kneel to perform physical assessment
  • Reliable transportation to complete home visit assessments 

 

Preferred Qualifications:

  • 1+ years of clinical experience in their highest level of education, clinical setting
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home Health care or home visit experience
  • Ability to travel up to 5% within licensed states if requested for additional incentives (with notice, based on business needs)
  • May be requested to obtain additional licensure in other geographic areas
  • Fluent in any languages besides English

 

Compensation for this specialty generally ranges from $125,000 - $175,00. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. Extra shift pay if you elect to pick up additional shifts 1:1 patient ratio; see 4-7 patients per shift Competitive compensation with quarterly and annual bonuses Comprehensive benefits, including CME and license assistance Onsite training and growth opportunities Company vehicle, malpractice, and liability coverage What You’ll Do Perform comprehensive health assessments and diagnose mild to complex illnesses Provide therapeutic interventions including splinting, suturing, wound care, minor procedures, IV fluids, and medication administration Order, perform, and interpret point‑of‑care laboratory studies Analyze diagnostic data to guide treatment plans and ensure continuity of care Educate patients and families on condition management and recovery Navigate difficult conversations related to goals of care and end‑of‑life decisions Communicate effectively with patients, families, medical power of attorney, primary care providers, and care partners Accurately and thoroughly document all patient encounters Adhere to evidence‑based clinical standards, safety protocols, and performance metrics Attend required training sessions and clinical team meetings Drive a DispatchHealth‑branded vehicle to patient homes and deliver care independently Perform additional duties as assigned What You’ll Need Current, unrestricted NP or PA license in the state of practice National board certification (current) Minimum 2 years of APP experience in Emergency Medicine, Urgent Care, Internal Medicine, Hospital Medicine, Family Practice, Cardiac, Pulmonary, or related settings BLS and ACLS certification Prescriptive authority and active DEA (as required by state) Valid driver’s license with clean driving record. Continued employment is contingent upon maintaining an acceptable driving record in accordance with company policy and insurer requirements.
