  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.

Required Certifications and Licensures:

  • Licensed to practice as a registered nurse in the state of agency operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.

Part time Registered Nurse Case Manager Anticipated Territory : Petaluma Area Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day) Also willing to consider Full Time: Saturday and Sunday plus 3 week days. What you’ll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
