
Medical Assistant - Outreach Specialist
Job Description
As part of the Saint Luke's Physician Group (SLPG) Population Health Division, the Medical Assistant - Outreach Specialist collaborates with internal and external stakeholders to help optimize health management and improve patient outcomes through proactive outreach, care coordination, and patient engagement. This position plays a vital role in connecting patients to needed care, supporting quality initiatives, and closing gaps in care that benefit both patients and payors.
The Medical Assistant - Outreach Specialist is responsible for identifying patients who require follow-up care after hospital or emergency department visits, coordinating outreach efforts, scheduling appointments, and assisting patients with preventive health needs. The majority of work is performed through telephone outreach, Epic documentation, and electronic communication with patients, providers, and care teams.
This role requires a highly motivated and organized individual who can work independently, manage competing priorities, and effectively engage patients while supporting organizational quality and population health goals.
Location: Blue Springs Primary Care
Schedule: Full-Time | Monday-Friday | Day Shift
Hybrid Opportunity: Eligible to work from home up to 3 days per week after successful completion of the first 90 days and meeting productivity expectations.
Primary Responsibilities
- Review reports from external healthcare facilities to identify patients recently discharged from hospitals and emergency departments.
- Coordinate and assign discharge follow-up activities to the appropriate care team members.
- Conduct emergency department (ED) follow-up calls for low-risk patients.
- Assist patients in scheduling follow-up appointments with their primary care provider.
- Conduct outreach to support care gap closure initiatives, including:
- Scheduling annual wellness visits.
- Coordinating laboratory appointments.
- Encouraging completion of preventive screenings and exams, such as diabetic eye exams.
- Accurately document patient interactions and care activities within Epic.
- Collaborate with providers, clinic staff, and care teams to support patient care and population health objectives.
- Maintain productivity, quality, and service standards while working independently in a hybrid environment.
