As an Outpatient Dialysis Registered Nurse (RN), you will provide compassionate, high-quality care to patients receiving hemodialysis treatment. You will play a key role in monitoring patient health, educating patients and families, coordinating care, and ensuring safe and effective dialysis treatments.

This role is ideal for nurses who enjoy building relationships, providing patient education, and developing expertise in a specialized area of nursing.

Qualifications

Required

  • Graduate of an accredited School of Nursing
  • Current Registered Nurse (RN) license
  • Current BLS certification (or ability to obtain)
  • Meet applicable state nursing practice requirements

Preferred

  • One year of RN experience
  • Hemodialysis, nephrology, or chronic care experience

Compensation

Hourly Rate: $29.00–$50.00/hour (based on experience, location, skills, and qualifications)

Nurse Assessment Coordinator - Continuing Care Center

MAIN FUNCTION Responsible for pre-admission screening, determination of skilled coverage and overseeing accurate Minimum Data Set (MDS) coding and development of the Interdisciplinary Plan of Care. Additionally this individual would be responsible for the coordination of the Medicare determination and certification requirements. Works closely with the Director of Clinical Compliance and the DON to support the mission and strategic imperatives of Thompson Health.
