Job Summary:

As a Registered Nurse (LVN/LPN), you will play a vital role in our skilled nursing and rehabilitation center, assisting in the recovery of patients from surgery, injury, or illness. We are seeking a compassionate and skilled nurse to join our team, working a double weekend shift schedule. Responsibilities:

  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information

  • Develop and implement individualized care plans, working closely with the interdisciplinary care team

  • Monitor patient progress, identifying areas of improvement and making adjustments to care plans as needed

  • Collaborate with other healthcare professionals to ensure seamless communication and coordination of care

  • Participate in quality improvement initiatives, contributing to the development of evidence-based practices and protocols

  • Maintain accurate and timely records, adhering to HIPAA guidelines and organization policies
