About the Role:

Lead a team of dedicated healthcare professionals to provide exceptional patient care and services
Develop and implement individualized care plans that address the unique needs of each patient
Collaborate with other departments, such as physical therapy, occupational therapy, and speech therapy, to ensure comprehensive care
Monitor patient condition and progress, adapting care plans as needed
Provide education and guidance to patients and their families on disease management, self-care, and discharge planning
Ensure compliance with policies, procedures, and regulatory requirements

Responsibilities:

Oversee and coordinate the daily activities and operations of the nursing team
Provide direct patient care, including administering medications, performing treatments, and assessing patient condition
Conduct initial and ongoing assessments of patients, including physical, emotional, and social evaluations
Develop and implement discharge plans that promote patient safety and independence
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Maintain accurate and detailed records of patient care and progress
