About the Role:
As a Licensed Practical Nurse LPN, you will be responsible for delivering compassionate and individualized care to our patients. Your duties will include:
Providing direct patient care, including administering medications, wound care, and other treatments
Developing and implementing patient care plans in collaboration with the interdisciplinary team
Monitoring and reporting patient progress to medical staff and families
Participating in quality improvement initiatives to ensure optimal patient outcomes
Demonstrating a strong commitment to patient-centered care and customer service
Responsibilities:
Collaborate with interdisciplinary teams to develop and implement individualized patient care plans
Administer medications and treatments as prescribed by physicians and other healthcare professionals
Monitor and report patient progress to medical staff and families
Participate in quality improvement initiatives to ensure optimal patient outcomes
Demonstrate a strong commitment to patient-centered care and customer service
Maintain accurate and up-to-date patient records and documentation
Minimum Requirements:
Current LPN licensure in the State of New Jersey
High school diploma or equivalent required
Minimum 0 years of experience in a healthcare setting
Ability to lift, transfer, and move patients as needed
