Job Overview

Fairview Range is looking for a Registered Nurse II to join our Oncology Department at our Fairview Range Hibbing Clinic in Hibbing, MN. 

 

The RN II will focus on care management as well as identifying and managing patient outcomes across the continuum. A Care Coordinator is a registered nurse who manifests a high level of expertise to assess, plan and facilitate treatment for actual or potential complex health problems of individuals, families or communities. The Oncology Care Coordinator coordinates and manages the care of the Oncology patients in order to meet multiple service needs, ensure continuity of services, decrease fragmentation of care in all settings, enhance the patient's quality of life, and ensure cost containment. The care coordinator assists the patient/family in coping with illness, optimizes the patient's self-care abilities, and supports patient's rights to make choices. In addition to managing patients through an episode of illness, the care coordinator focuses on wellness and disease management. The Oncology Care Coordinator has knowledge and skill to provide IV/VAD Care as well as administer medications through all routes in an outpatient setting.

 

Schedule: This position is full time, working 72-80 hours per two weeks. 8 - hour day/evening shifts and weekends as needed.

As a Registered Nurse II (RN II), you will be responsible for:

  • Assessments
  • Performing a total systems assessment according to established policies. Timely and accurate assessment of the patient's clinical, social, functional, and continuing of care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention.
  • Collaborating with the interdisciplinary project team to develop clinical pathways.
  • Working collaboratively with the Oncology care management team to ensure coordination of the care of patients across an episode or continuum.
  • Establishing a relationship with patients and their family members for the purpose of advancing and coordinating care.
  • Assessing community, institutional, and family support systems and resources.
  • Coordinating a plan of care through an interdisciplinary and collaborative group practice in conjunction with the patient, family and care givers.
  • Facilitating advancement of the interdisciplinary plan of care through communication with all involved disciplines to promote coordination of care and delegation of nursing care to associates.
  • Documenting patient's progress toward goal achievement and patient outcomes.
  • Educating the patient and members of the healthcare delivery team about healthcare and treatment options, community resources, insurance benefits, and psychosocial concerns.
  • Planning for and identifies immediate, short term, and long-term care needs for patient/client. Sets appropriate patient/client goals and time frames for achievement in order to achieve desired outcomes
  • Demonstrating knowledge and understanding of the client's diagnosis, prognosis, care needs, as well as cost and barriers to goal achievement. Written plan of care/ documentation reflects appropriate actions to meet expected outcomes.
  • Demonstrating ability to communicate appropriate criteria to payers to ensure appropriate level of reimbursement. Demonstrating operational use of criteria for continued stay and admissions reporting.
  • Maintaining an individualized plan of care, related to the patients' needs in collaboration with the physician, patient and significant others. Determines appropriate utilization of resources based on clinical assessment, guidelines and standards of care.
  • Continually reassess the patient's health status, environment, and support system with ongoing comprehensive assessment. Plans maximize independence of patient/caregiver. Evaluates the patient's progress and response to care/services delivered and adjusts the plan accordingly.
  • Continualling reassess resource utilization as patient progresses towards goals. Reviews, prioritizes and implements POC based on payer limitations.
  • Determining appropriate utilization of resources based on clinical assessment, guidelines and standards of care.
  • Providing age specific direct patient care in collaboration with all disciplines according to standards of care.
  • Care manager evaluates and measures patient response to planning, services, and interventions in order to assure quality of case management services and to make improvements where necessary.
  • Participating in the evaluation of care process to identify variations and opportunities for improvement.
  • Directing subordinates in the implementation of care.
  • Providing appropriate nursing care based on assessments and in coordination with medical care, including external referrals, as directed.
  • Preparing and administers appropriate treatments, testing, and medication (including high risk drugs), as ordered by the physician.
  • Coordinating patient schedules with physicians and other departments to ensure timely appointments.
  • IV/VAD Maintenance & Administration of Medications
  • Administration of Chemotherapy, biotherapies and other medications associated with the care of Oncology and community infusion patients.
  • Knowledge of drugs and side effects.
  • Safe administration of chemotherapy, biotherapy, hazardous drugs and blood products.
  • Safe administration of IV solutions and medication additives.
  • Knowledge of various venous access devices.


Required Qualifications

  • Associates Degree in Nursing
  • 1 year acute, outpatient and/or ambulatory direct patient care RN experience or
  • 1 year recent oncology experience
  • MN Registered Nurse (RN) License
  • Basic Life Support (American Heart Assoc or Red Cross) within 90 Days
  • ONS-Chemo Immuno Cert within 180 Days

Preferred Qualifications

  • Bachelor of Science Nursing
  • Previous case coordination experience
  • 1 year clinical experience in the Oncology care setting
  • Oncology Certified Nurse

Job Overview Fairview is hiring a Registered Nurse (RN) for our Oncology-5A Department at our M Health Fairview University of Minnesota Medical Center - East Bank location in Minneapolis, MN . As a Registered Nurse in our Oncology unit, you would help adult patients and oncologic malignancies. Our services include hematologic malignancy, gynecological oncology, solid organ tumor oncology, colorectal, plastic surgery, and gender affirmation surgery. Nurses are responsible for providing chemotherapy, radiation, and end-of-life care. We deliver complex medication and infusion therapies, blood administration and care for patients undergoing diagnostic and therapeutic surgeries, procedures, and interventions on the unit. You would use the nursing process to provide for the physical, emotional, and educational needs of adult patients undergoing hematological, oncological, and surgical therapy. You would also manage complex medication and infusion therapies as well as safely administer medications and infusions in accordance with relevant policies and guided by knowledge of the indications, contraindications, side effects, normal dosages, and method of delivery. Schedule: This position will be working 0.6 FTE (48 hours every two weeks). The shift length will be 8 hours on the evening shifts (evenings: 3:00pm-11:30pm). This position will also work a rotation for every other weekend. About Our Oncology-5A Department at UMMC East Bank : Oncology 5A is a new unit that has combined both our previous Surgical and Medical Oncology units. We have a 39-bed capacity. We are looking for a nurse who is eager to learn new skills, has a high level of communication and critical thinking, and who is prepared for high levels of acuity. Our Registered Nurses must achieve or maintain an oncology-specific certification (e.g., OCN) or complete the oncology-specific continuing education credit requirement as defined by Commission on Cancer Accreditation standards. As an Oncology RN, you will be responsible for: Assessing patient needs and identifying expected nursing outcomes; Developing an individualized nursing plan of care; Implementing interventions as needed during treatment based on the plan of care; Evaluating care and patient response to interventions and expected outcomes; Using communication strategies to achieve desirable outcomes; Coordinating care with members of the health care team and delegating responsibilities to others as appropriate; Demonstrating the ability to advance care by utilizing standards, guidelines, care pathways, and continuous improvement to enhance patient outcomes; Promoting a culture of evidence based practice (EBP) by participating in a multitude of available opportunities; and Performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act.
