Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

  • Must be a graduate of an approved school of clinical education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed professional.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
  • Medicare home health or hospice experience is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider

The clinician should possess the ability to delegate visit responsibility to the clinicians’ on-call. Or when indicated, to notify patient to seek emergent care via hospital emergency room. Instructs and counsels the patient and caregiver regarding care requirements of the patient in the home. The Triage Nurse will enter a note into the Case Communication note as an On-Call Note to document the communication between the Triage Nurse and the patient. The documentation will include the clinician’s instructions to the patient and the visiting clinician and/or the patient primary field clinician. The Triage Nurse will also document the clinicians name and time directive was given. The Triage Nurse will be responsible to complete the intake process for after hour referral calls.
