As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.  Depending on your strengths and interests, we have various nursing opportunities available.

Qualifications

Reviews daily patient reports and assignments with Nursing Assistants upon the beginning of the assigned shift. • Assures that the unit area is maintained in a clean and safe manner for resident comfort and convenience. • Conducts daily resident rounds to assess and evaluate the resident’s physical, medical and emotional status and to implement or revise nursing interventions to the resident plan of care. • Records care information accurately, timely and concisely. Completes all required documentation including resident assessments, interventions, and patient response(s) in the medical record in accordance with policy. • Initiates, reviews, revises, and updates resident Plans of Care as indicated.
