About Greenwood Center

Greenwood Center is seeking compassionate, dependable, and dedicated Licensed Practical Nurses (LPNs) to join our nursing team in Warwick, Rhode Island. As an LPN, you will provide high-quality nursing care to residents in a skilled nursing environment. You will work alongside registered nurses, physicians, and other healthcare professionals to promote residents' health, comfort, dignity, and independence. We are looking for nurses who demonstrate sound clinical judgment, communicate effectively, and are committed to delivering compassionate, resident-centered care.

Responsibilities

Provide nursing care in accordance with physician orders, individualized care plans, facility policies, and applicable regulations.
Administer medications and treatments as prescribed and within the scope of practice.
Monitor residents for changes in condition and promptly report concerns to the appropriate healthcare professionals.
Perform nursing duties, including vital sign monitoring, wound care, and other treatments within the scope of practice.
Maintain accurate and timely documentation of resident care.
Collaborate with RNs, physicians, and interdisciplinary team members to support residents' health and well-being.
Participate in care plan updates and resident care conferences as needed.
Communicate effectively with residents, families, and healthcare team members.
Promote residents' dignity, comfort, privacy, and independence.
Follow infection control procedures, safety standards, and facility policies.
Respond promptly and appropriately to residents' needs and changes in condition.
Support a safe, respectful, and collaborative care environment.

Qualifications

Graduate of an approved Licensed Practical Nursing (LPN) program.
Current, active Rhode Island LPN license in good standing required.
Current CPR certification required.
Previous long-term care or skilled nursing experience preferred, but not required.
Strong communication, clinical judgment, and teamwork skills.
Ability to prioritize responsibilities and respond appropriately to residents' needs.
Dependable, compassionate, and committed to providing high-quality resident care.
