Strong Clinical Skills. Confident Leadership. A Team That Needs You.

Are you an experienced LPN leader who knows how to bring structure, accountability, and teamwork to a clinical department?

Do you enjoy developing caregivers, solving problems, and making sure residents receive exceptional care?

Charter Senior Living of Woodholme Crossing is seeking a Director of Nursing (DON) / Health & Wellness Director to lead our clinical and resident-care operations!

We're looking for a self-motivated, organized, and hands-on nursing leader who can confidently manage clinical responsibilities while building a strong, dependable team.

This isn't a position for someone who wants to manage exclusively from behind a desk.

We need a leader who is visible, approachable, clinically confident, and willing to step in alongside the team when needed.

If you're passionate about senior living, enjoy developing employees, and take pride in creating a well-organized, resident-focused clinical department, we'd love to meet you!

What We're Looking For

Clinical Expertise • Strong Communication • Hands-On Leadership

Clinical Expertise

You bring strong nursing fundamentals, sound clinical judgment, and the ability to oversee resident assessments, care planning, medication management, and changes in resident condition.

Strong Communication

You know how to communicate effectively with residents, families, caregivers, physicians, and community leadership. You can navigate difficult conversations, address concerns professionally, and ensure important information doesn't fall through the cracks.

Hands-On Leadership

You lead by example. You can organize a department, establish clear expectations, coach employees, hold the team accountable, and jump in to help when needed.

What You'll Be Responsible For

Clinical Operations & Resident Care

  • Oversee daily nursing, wellness, and resident-care operations.

  • Conduct resident assessments and reassessments to ensure appropriate levels of care.

  • Develop, review, and maintain individualized resident service and care plans.

  • Monitor resident conditions and ensure timely follow-up when changes occur.

  • Oversee medication administration practices, medication documentation, and medication safety.

  • Coordinate care with physicians, pharmacies, outside providers, residents, and families.

  • Ensure accurate and timely clinical documentation.

  • Support infection prevention, quality assurance, and resident safety initiatives.

  • Maintain a compassionate, resident-centered approach to care.

Leadership & Department Accountability

  • Lead, coach, mentor, and support nursing and caregiving associates.

  • Build structure, organization, and consistency within the clinical department.

  • Establish clear expectations for resident care, documentation, communication, and staff responsibilities.

  • Support employee hiring, onboarding, orientation, scheduling, and training.

  • Address performance concerns and provide constructive feedback.

  • Promote teamwork, accountability, and professional communication.

  • Help identify staffing needs and prioritize department responsibilities.

  • Serve as a visible clinical resource and support team members when challenges arise.

  • Step in to assist with resident-care responsibilities when needed.

Compliance & Operational Excellence

  • Maintain compliance with applicable Maryland assisted living regulations, clinical standards, and company policies.

  • Support regulatory inspections, survey readiness, and corrective-action initiatives.

  • Identify opportunities to improve clinical processes and department efficiency.

  • Maintain organized resident records and clinical documentation.

  • Partner with the Executive Director and other department leaders to support resident satisfaction and operational goals.

  • Address clinical concerns proactively and follow through on solutions.

  • Help maintain a safe, organized, and resident-focused care environment.

Qualifications

Required

  • Active LPN license with authorization to practice in Maryland.

  • Previous professional nursing experience with strong core clinical skills.

  • Ability to perform resident assessments, oversee care planning, and manage clinical documentation.

  • Strong interpersonal and communication skills.

  • Excellent organization, time management, and multitasking abilities.

  • Ability to lead, support, and provide direction to nursing and caregiving associates.

  • Confidence handling multiple priorities in a fast-paced environment.

  • Flexibility and willingness to assist with hands-on resident care when needed.

  • Availability for the required on-call rotation and rotating weekends.

Preferred

  • Previous experience as a Director of Nursing, Health & Wellness Director, Resident Care Director, Assistant Director of Nursing, or nursing supervisor.

  • Assisted living, memory care, or senior living experience.

  • Experience supervising, training, and developing caregivers or nursing associates.

  • Experience establishing clinical workflows, improving department organization, and reinforcing accountability.

  • Familiarity with Maryland assisted living regulations and compliance requirements.

Who Will Thrive at Woodholme Crossing?

We're looking for someone who:

  • Is a self-starter who takes initiative without waiting for direction.

  • Can bring organization, structure, and consistency to a clinical department.

  • Has strong clinical assessment and nursing skills.

  • Communicates clearly and professionally with residents, families, and employees.

  • Can multitask, prioritize, and remain calm under pressure.

  • Is flexible and adaptable when resident or staffing needs change.

  • Knows how to coach employees while maintaining accountability.

  • Builds trust with caregivers and leads through example.

  • Is willing to jump in and help the team rather than simply delegate.

  • Takes ownership of problems and follows through until they're resolved.

  • Understands the importance of teamwork, resident satisfaction, and regulatory compliance.

  • Leads with compassion, professionalism, and confidence.

We want a strong nurse, a strong communicator, and a dependable people leader who can bring the entire wellness department together.
