Responsibilities:

Provide direct patient care to clients in their homes, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming
Monitor and report changes in client condition
Medication reminder
Maintain accurate and detailed records of client care and services provided
Demonstrate a commitment to excellence in patient care, professionalism, and customer service

Requirements:

High school diploma or equivalent required
Ability to work in a fast-paced, dynamic environment
Excellent communication and interpersonal skills
Reliable transportation and a valid driver's license
