BMT and Cell Therapy RN Coordinator

The Stem Cell Transplant (SCT) RN Coordinator is responsible for coordination of the evaluation and preparation of patients for Hematopoietic Stem Cell (HSC) transplantation and cellular therapies. The SCT RN Coordinator is responsible for coordinating patient care across multiple areas 'the outpatient clinics, the inpatient nursing unit and the community ' in order to provide continuity of patient care across settings. Problems encountered have a significant level of complexity due to both clinical and logistical issues. Independent thinking and a high degree of problem solving and innovation are required to resolve complex and unanticipated problems, applying appropriate interventions to individual patients and patient problems while performing daily tasks. The SCT RN Coordinator is also expected to serve as a clinical resource for other team members, patients/family/caregivers, referring case managers and community clinicians. Although supervision and direction are provided by the Cellular Therapy Program Manager and physicians, the SCT RN Coordinator has wide latitude in decision making within written organizational and department policies and principles. A wide variety of internal and external relationships are involved in performance of the SCT RN Coordinator's duties: - Interacts daily with patients, families/caregivers, nurses, pharmacists, social workers, discharge planners and physicians to provide and facilitate care. - Communicates and collaborates with community physicians, pharmacists, apheresis department, cellular therapy lab, advance practice providers, financial coordinators, insurance company case managers and medical directors to ensure a successful continuum of care. - Maintains a current and ongoing list of cell therapy patients. Organizational skills and problem solving abilities are paramount. In addition to independently planning and scheduling their own daily schedule, the coordinator must be able to prioritize clinical issues and case manage a large number of patients in various stages of the transplant/cellular therapy processes. Additionally, the SCT RN Coordinator must integrate the clinical responsibilities noted above, provide assistance and guidance to other program staff, and participate in program development, and educational activities. This is a non-benefited temporary/seasonal position. Job Description The CNA provides direct resident care in accordance with facility policies, procedures, and standards. Responsibilities include providing personal care, assisting with activities of daily living, making beds, turning and repositioning residents, obtaining and recording vital signs, and observing and reporting changes in residents’ conditions. The CNA is responsible for completing required documentation, promptly reporting accidents, injuries, or changes in condition, and answering call lights regardless of assignment. Required Qualifications Valid Connecticut CNA certification in good standing Minimum 2 years of experience in senior care/skilled nursing PCC proficient No history of patient/resident abuse as reported on the State Registry Ability to read, write, and follow oral and written instructions Strong attention to detail and ability to prioritize multiple tasks Reliable, dependable, and able to work well as part of a team Demonstrates patience, professionalism, enthusiasm, and a positive attitude toward residents and their support networks Flexibility to work various shifts as needed Attendance is a MUST. Two call-outs, in accordance with the handbook, are considered a termination of the temporary contract. The Home Health Aide is a certified individual who provides direct patient care (personal care) as designated by the Registered Nurse Case Manager, Physical Therapist, Occupational Therapist, or Speech Therapist. Gives competent and safe patient care, i.e. bathing, dressing, oral, toileting, etc, according to the home health aide plan of care established by the RN Case Manager or Therapist. 2. Meets the needs of complex patients, i.e. bedbound, palliative and hospice patients, etc 3. Provides safe techniques for ambulation, transfers, and use of assistive devices. Uses proper body mechanics in positioning and movement of patients. 4. Assists with range of motion exercises as directed and also with exercise programs instructed by the nurse or therapist. 5. Assists with meal preparation in accordance with prescribed diet. Understands fluid restrictions, recording, and feeding. 6. Addresses clean up and maintenance of the patient's immediate environment. 7. Communicates any untoward patient findings to the RN Case Manager or therapist. 8. Follows universal precautions and appropriately uses personal protective equipment. 9. Documents care rendered and observations as instructed by RN Case Manager or Therapist. 10. Ensures that all patient related documents are signed and dated and handed in timely. 11. Conforms to payroll standards. 12. Complies with HIPAA. 13. Complies with weekday and weekend commitments. 14. Participates in staff meetings, in-service programs, and all other required meetings. Qualifications EDUCATION High School education or GED preferred, but not necessary. EXPERIENCE Certified according to State of Connecticut Public Health Code regulations. LICENSURE Successful completion of a home health aide training program approved by the State of Connecticut Department of Health or a student LPN or RN who has successfully completed 75 hours of theoretical and clinical instruction in the fundamental skills of nursing approved by the CT DPH. Successful completion of a home health aide competency evaluation program approved by the State of CT DPH Successful completion of a new competency evaluation program, if there has been a consecutive period of 24 months that the home health aide has not performed nursing-related services for monetary compensation. All Home Health Aides are required to have a valid CT driver's license, a clean driving record, and a car available for work purposes as a condition of employment. SPECIAL SKILLS Demonstrates excellent communication and interpersonal skills. Must be adaptable, flexible, ability to learn quickly, and have a comfort level in providing care in patient homes. PHYSICAL DEMAND Must be able to lift objects in excess of 50 pounds. Must be able to perform tasks that require routine standing, bending, and lifting.
