Job Summary:

Responsibilities:

  • Provide direct patient care under the supervision of licensed nurses, including vital sign monitoring, assistance with activities of daily living, and basic wound care

  • Assist with eating, dressing, grooming, and bathing patients, ensuring their comfort and dignity

  • Support patients with mobility issues, providing transfers, ambulation, and range-of-motion exercises as needed

  • Monitor and report any changes in patient condition to licensed nurses, maintaining accurate records of patient care

  • Collaborate with multidisciplinary care teams to develop and implement personalized care plans

  • Maintain a clean and safe environment, adhering to infection control policies and procedures

  • Participate in ongoing education and training to expand knowledge and skills in patient care
