As an RN Clinical Liaison at Crossroads Hospice, you’ll be the reassuring presence families often meet at one of the most difficult moments of their lives. You serve as both an educator and a compassionate guide, helping patients and their loved ones understand hospice care while offering emotional support with empathy and grace.

Registered Nurse Liaison (RN) Qualifications:

  • 1+ years of prior Registered Nursing experience in a setting with exposure to end-of-life and/or palliative patients. 
  • An active unencumbered state RN license
  • Current driver's license and auto liability coverage
  • CPR certification, a copy of TB Results, a copy of an updated physical
  • Strong understanding of Hospice and Palliative care

Your responsibilities may include: Providing skilled nursing care based on each client's individualized care plan Completing comprehensive nursing assessments and reassessments Performing OASIS assessments and required documentation Completing Start of Care, Resumption of Care, Recertification, and Discharge assessments Monitoring clients' conditions and identifying changes in their health status Developing, updating, and evaluating individualized plans of care Coordinating care with physicians, therapists, caregivers, clients, and families Providing education and support to clients and their families Performing wound care and other nursing procedures within your scope of practice Administering medications and treatments as ordered Maintaining accurate and timely clinical documentation Helping clients maintain their comfort, dignity, safety, and independence Following applicable nursing standards, Medicare requirements, and agency policies What We Offer Competitive Point-Based Compensation $45 per point $50 per day for on-call rotations $0.43 per mile for eligible mileage.
