Job Summary:

We are seeking a compassionate and dedicated Home Caregiver to join our team at SYNERGY HomeCare in Bedford, NH. As a Home Caregiver, you will have the opportunity to make a meaningful difference in the lives of our clients by providing high-quality, personalized care in their own homes. Responsibilities:

Provide non-medical care and assistance to clients in their homes, including personal care, light housekeeping, and meal preparation
Assist clients with daily living activities, such as bathing, dressing, and grooming
Monitor and record client vital signs and report any changes to the care team
Assist clients with medication management and daily routines
Facilitate socialization and companionship to promote emotional well-being
Provide emotional support and companionship to clients and their families
Collaborate with the care team to develop and implement personalized care plans
Maintain accurate records and reports of client care and services provided
