Job Summary:

Carease Health - OH, a leading nationwide preventative healthcare company, is seeking an experienced Adult Nurse Practitioner to join our team in Napoleon, OH. As a key member of our clinical team, you will deliver high-quality preventative care to residents in skilled nursing facilities through a combination of on-site visits and remote work from home. With a strong background in Geriatrics or Internal Medicine, you will build strong relationships with facilities, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

Provide comprehensive preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home
Develop and implement individualized care plans, utilizing remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM) principles
Conduct assessments, diagnose and treat acute and chronic conditions, and order and interpret diagnostic tests
Collaborate with healthcare teams to ensure seamless care coordination and communication
Maintain accurate and detailed records, utilizing a dedicated scribe team for efficient documentation
Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines

Requirements:

Active Adult Nurse Practitioner license (in the state of practice)
2+ years of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Masters degree in Nursing or related field
Ability to work independently and as part of a team
Strong clinical judgment and critical thinking skills
