We are seeking an experienced RN MDS Coordinator to oversee resident assessments and care planning in our skilled nursing and rehabilitation facility. Join our interdisciplinary team to support accurate documentation, appropriate reimbursement, and quality resident care.

Key Responsibilities:

  • Coordinate accurate, timely completion and submission of MDS assessments, CAAs, and care plans.

  • Manage assessment schedules and support Medicare, Medicaid, and managed care requirements.

  • Collaborate with nursing, therapy, residents, and families to develop individualized care plans and support safe discharge planning.

  • Participate in utilization review, Triple Check, and quality improvement activities.

  • Educate team members on assessment processes and documentation requirements.

Qualifications:

  • Active RN license in the state of employment.

  • Bachelor’s degree in nursing. Preferred

  • Proficiency in MDS 3.0 and knowledge of the RAI process.

  • At least two years of clinical experience in long-term care.

  • Prior experience as an MDS Coordinator.

  • Strong organizational skills and knowledge of applicable state and federal regulations.
