The Gardens at Country, a leader in senior living care, is seeking a highly skilled and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our team. As a valued member of our healthcare staff, you will be responsible for providing exceptional care to our residents while working collaboratively with our interdisciplinary team.

Responsibilities:

• Provide direct patient care to our residents, including administering medications, wound care, and vital sign monitoring
• Collaborate with the healthcare team to develop and implement individualized care plans
• Conduct resident assessments and document findings accurately and thoroughly
• Participate in quality improvement initiatives to ensure high-quality care
• Maintain accurate and up-to-date records of resident care and services
