JOB SUMMARY
The CommUnityCare Centering Pregnancy and Centering Parenting programs offer an alternative care model
focused on group visits and education, which promotes positive health outcomes and patient satisfaction for
pregnancy and pediatric care. These programs are nationally accredited by the Centering Healthcare Institute.
The Medical Assistant - Group Facilitator serves as co-facilitator of CommUnityCare's Centering Pregnancy and
Parenting programs. The Medical Assistant Centering Facilitator works in close collaboration with providers, clinical
staff, and the Centering Steering Committee in order to ensure adherence to program operations and standards.

The CommUnityCare Diabetes Self-Management Training (DSMT) program offers an alternative care model focused
on group visits and education, which promotes positive health outcomes and patient satisfaction for patients living
with diabetes. These programs are nationally accredited by Association of Diabetes Care & Education Specialists
(ADCES).

The Medical Assistant – Facilitator serves as co-facilitator of CommUnityCare’s DSMT program. The Medical
Assistant Group Care Facilitator works in close collaboration with providers, clinical staff, and the certified diabetes
care & education specialists to ensure adherence to program operations and standards. The facilitator will be cross
trained to facilitate and support group care programs across multiple specialties based on program needs.

Responsibilities:

JOB FUNCTIONS
Essential Functions
• Co-Facilitates with licensed provider Centering Pregnancy and/or Parenting groups and/or diabetes groups or
other specialties and discussions.
• Preparation of centering groups including chart preparation and organizing materials and supplies for each group
and session. Responsible for managing supplies and inventory.
• Complete all direct and indirect care documentation in timely manner and ensure patient records are current and
complete.
• Oversight of recruitment, participation, retention and direct point of contact for patients in Centering Pregnancy
and Centering Parenting groups and diabetes groups or other specialties.
• Responsible for reporting information, assist with monitoring and maintaining the program's database to reflect
participant delivery outcomes, benchmark data, and other indices as specified through awarded grants.
• Collaborate with key players to ensure coordination and oversight of programming schedules.
• Communicate and work in conjunction with other CommUnityCare centering sites. Back up facilitator for other sites
as needed.
• Collaborate with direct supervisor and co-facilitator (provider lead) to assess, create and introduce new program
curriculum.
• Assist direct supervisor with community outreach as needed, including but not limited to managing social media
platforms, email correspondence and community events.
• Maintain all licensure and/or certificate requirements.
• Support direct supervisor with the implementation of program to new sites and the site approval processes.
Perform other duties as assigned.
• Attend all practice leader and /or clinic site meetings regularly. These may be related to quality assurance, patient
care and other related topics.
• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties
comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare
• Standard Operating Procedures and CommUnityCare Policies and Procedures.
• Travels between CommUnityCare sites as needed to facilitate and support group care programs.

Qualifications:

Minimum Education:

• Required/Preferred And/Or High School Diploma or equivalent (higher degree accepted).

• Required Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience.

Work Experience:
• Required completion of a medical assistant externship.

• Preferred experience with DSMT, diabetes education, or group based chronic disease education.

• Previous facilitator experience preferred.

Required to meet one of the following: Medical Assistant experience, or CNA/EMT/PCT(or other comparable experience) providing direct patient care, beyond Activities of Daily Living, including experience taking manual vitals and giving medications and/or injections, or Graduate of a medical assistant program. Preferred: Six (6) months or more experience as a Medical Assistant. Certificate/License Medical Assistant certification preferred. Current AHA approved Basic Life Support (BLS) Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card or within 60 days of hire required. Knowledge, Skills and Abilities Ability to administer medications & injections within scope of position within 90 days of hire. Knowledge of medical terminology. Excellent organizational skills.
